Chair-side bisacryl temporaries flex, discolor, and rarely capture the esthetic intent of the final restoration. Lab-milled PMMA provisionals are dimensionally stable, translucent, polishable, and can be fabricated to the exact shade and form of the planned final restoration — so the patient wears a true smile preview during the healing phase.
For full-arch cases, we fabricate immediate provisionals from the diagnostic wax-up or digital design so they are ready for surgery day delivery.

Common questions from referring dentists about our milled PMMA provisionals.
When is a lab-milled provisional worth it over a chairside temp?
Any time the provisional has to do real work. Milled PMMA is denser and more accurate than a chairside temporary, holds its shape and shade, and stands up to being worn while tissue heals or while a full-mouth case is worked through. If the provisional is also the esthetic blueprint for the final restoration, it is worth milling.
Can a provisional be used to test a design before we commit to the final?
That is one of the best uses for it. We can mill the provisional straight from the approved diagnostic wax-up or digital design, so the patient wears the proposed shape and you see it in function. Whatever you learn from it gets carried into the final restoration.
Single units through full arch?
Both, including implant cases. Single units, quadrants and full-arch provisionals — the same workflow scales up, and full-arch provisional work is routine for us.
What do you need from me?
A scan or impression of the preparations and the opposing arch, plus a bite. If you want the provisional to follow a specific shape, send the wax-up or the pre-op records and we will mill to that rather than to an average.
Can I send a digital scan instead of an impression?
Yes. If your scanner supports it, add GIE as a connected lab and send cases straight from your portal — see connect your scanner. If your system isn’t supported, upload STL or PLY files directly through send your scan files. Conventional impressions are also accepted — call and we’ll send a FedEx label for pickup.
How long can a milled PMMA provisional stay in?
Four to six months is what we’d generally say, though it genuinely depends on the patient — a heavy bruxer will get less out of one, someone with lighter function can go longer. For context, a 2025 retrospective analysis of 443 immediate full-arch implant provisionals reported 180-day survival of 92.4% for milled PMMA (Sobczak et al., Clinical Implant Dentistry and Related Research). If you expect a longer provisional phase than that, tell us when you send the case.
